In Network Provider


It is recommended that you contact your insurance company first to determine your deductible and co-payment.  This will give you an idea what your out of pocket expenses are before you begin treatment.  Dr. Thomas is a provider for numerous health insurance plans.  Your health insurance company determines your eligibility for payment of your health insurance claims.  Your co-pay and deductible are determined by contract between your insurance company and your employer.  If are self employed and purchased your healthcare benefits contact the agent or company that sold you health insurance. Fee schedules are available for those who do not have insurance coverage or those who request services not covered by their health insurance.

A number of clients have out of network coverage.  If your insurance company is not listed, first contact your insurance company and Dr. Thomas to determine if Dr. Thomas is a contract provider for your company.
